1. Purpose
This policy shall define the processes utilised to store information obtained by SMTV.
2. Policy Requirements
2.1 SMTV shall store all information electronically on secure databases.
2.2 SMTV shall meet all confidentiality obligations regarding information.
3. Implementation
3.1 This policy is in effect at all times in all aspects of operations.
3.2 This policy shall be reviewed at intervals not exceeding 12 months.
4. Penalties
4.1 Any intentional breach of this policy shall be deemed serious misconduct.
5. SMTV Recordkeeping Statement
SMTV shall store all necessary information electronically on secure database(s). SMTV shall meet all confidentiality obligations and seek approvals as necessary to use the information for internal/external reviews.
6. SMTV Recordkeeping Management
SMTV shall at all times maintain the expected level of confidentiality and trust with regard to all engagement information (technical, commercial or other). SMTV shall employ the following Recordkeeping management principles:
6.1 Information Access – All information stored for a particular engagement shall be accessible only to SMTV representative(s) nominated by SMTV and approved by the client.
6.2 Information Security – All information stored for a particular engagement shall be accessible via secure database using a level of security (e.g. password, or secure link).
6.3 Information Structure – SMTV shall utilise robust file structures to compartmentalise information within the native file system (see example in Annex A).
6.4 Information Currency – SMTV shall utilise dated file structure for supply of client information (see example in Annex).
6.5 Information Revision – SMTV shall utilise both alphabetical and numeric revision numbers for SMTV-generated documents as appropriate.
6.6 Information – communication – SMTV shall utilise the storage of emails as required for engagement directives (e.g. scope, milestones, payments etc.).
6.7 Information – Confidentiality – SMTV shall operate on a strictly confidential basis. (See Annex B – Confidentiality Agreement).
